If you have a complaint or appeal about the care that a patient has received, fill out the Appeals and Grievances Form and email or fax it to us:

You may use the EthicsPoint third-party hotline, the online secure reporting form or you may mail in a downloadable report.
After you complete the report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.
