Member ID Cards:

You will also receive an ID card that shows your providers that you are covered. You should keep your ID Card with you at all times. If you misplace your card, please call customer service so they can assist you in replacing it: 503-584-2150.

Keep your card with you at all times. You’ll need it for the following:

  • Medical appointments
  • Dental appointments
  • Counseling or other mental health appointments
  • Picking up prescription drugs
  • Emergencies
