MOMs-MOMs Plus (Medicaid Obstetrical and Maternal Services)
MOMs provides support to women with WVCH Medicaid coverage who are pregnant and at risk for drug and alcohol abuse and/or who are covered by WVCH with a child covered by WVCH who is at risk for drug and alcohol abuse. MOMs mentors may attend court, or DHS or medical visits with the member, and may assist in locating sober living and/or transition out of incarceration.
MOMs mentors support providers by encouraging members to make safe choices with respect to their health and the health of their new baby.
Members who can access these services:
Pregnant women covered by WVCH CCO A and/or B and women covered by WVCH with a child who is in need of additional support and have a drug and alcohol history.
EDIT (Emergency Department Intervention Team)
EDIT peer mentors support members identified as high ED utilizers who are covered by WVCH in appropriate use of medical, mental and chemical dependency services and access social and community supports to meet their needs.
EDIT mentors support providers by encouraging appropriate use of health care services, including establishing care with a primary care provider, maintaining healthy provider-patient relationships and providing transportation to and from appointments.
Family Support Program
Family Support peer mentors work with children with complex medical needs who are covered by WVCH. They support parents in getting the needs of their children met through community resources.
The mentors support providers by encouraging appropriate use of health care services, maintaining healthy provider-patient relationships and providing transportation to and from appointments. Mentors can also help with the complex medical needs of children with medication issues, medical equipment, prior authorizations, etc.
